Compliance Calendar - This calendar details the important dates that affect a retirement plan. This can be used to ensure the compliance of the plan with government required dates. Beneficiary Designation Form - Use this form to designate a beneficiary for your retirement or savings plan benefit. Benefit Election Form Under $5000, Over $5000 - Use this form if you have terminated from the plan, and you need to elect in what form your benefit will be paid. Different forms exist for distributions over $5,000 and distribution under $5,000. Election Not to Participate - This form is used by any employee wishing to not participate in the retirement plan. Pre-Retirement Survivors Benefit Explanation and Election to Waive Pre-Retirement Survivor Benefit - This form needs to be completed by all employees who want to waive the annuity option in the retirement plan. This form can be collected in conjunction with the Beneficiary Designation Form. In-Service Withdrawal Election - This form is to be used for the withdrawal of funds before the participant separates from service or has a distributable event. Hardship Withdrawal - Use this form to request a hardship withdrawal from your 401(k) plan. Keep in mind that your plan must allow this option in order for you to use this form. Loan Application - Use this form to request a loan from your plan. Keep in mind that your plan must allow this option for you to use this form. Special Tax Notice Regarding Plan Payments - Use this form if you have terminated from your plan and you need to read the special tax notice to participants. The plan is required by IRS to provide you with this notice, which explains the tax consequences of taking your benefit distribution in cash, versus rolling it over to an IRA or to another qualified retirement or savings plan. Death Benefit Distribution Forms: Surviving Spouse, Non Spouse - Upon the death of a participant, the surviving spouse and/or beneficiary must complete a distribution form. Salary Deferral Agreement - This form allows an employee to indicate the compensation they want deferred to the retirement plan. Postponement of Distribution - This form must be completed by any terminated participant who postpones his/her distribution from the plan. Qualified Domestic Relations Order Procedure Packet and Distribution Form - This packet and forms helps the plan sponsor to process the correct distribution to a former spouse that has been grated a portion of the participants account balance. |
